POSITION: Resource Navigator (Application Deadline: Dec 31, 2020)

Position Information 

OPPORTUNITY FOR IMMEDIATE HIRE, $27/hour, 30 positions available with potential opportunity to be promoted to supervisor role quickly.

Purpose of Position

Under the supervision of the Supervising Resource Navigator, performs remote/fieldwork in support of COVID-19 contact tracing, with alternative work schedules and/or compressed workweeks. Conducts COVID-19 interviews around resource needs using a trauma-informed, culturally respectful approach that builds trust and facilitates the free sharing of information. Connects clients with identified resources. Elevates complex cases to Resource Navigator Supervisors. Follows all scripts, policies, and procedures provided through training and complies with appropriate handling of confidential information related to case interviews and contacts. All work is conducted in a confidential manner. 

This is a full-time, temporary position. Can discuss/accommodate P/T hours if needed.

Examples of Typical Tasks: 

-Conducts telephone calls with persons diagnosed with COVID-19 and those identified as close contacts to assess resource needs for self-isolation and quarantine.

-Identify common needs amongst COVID-19 patients and search for local solutions. This could include access to GetFood, mental health services, legal or financial counseling, and other community programs helping patients in need and/or people in isolation or quarantine. 

-Connects clients with needed resources through various platforms. 

-Communicates with cases and contacts in a professional and empathetic manner. 

-Collects and records information obtained from the resource interview and gathering into the data systems in accordance with standard operating procedures. 

-Follows approved scripts and protocols, provides people with vetted information about local community resources. 

-Elevates complex cases to Resource Navigator Supervisors. 

-Maintains daily communication with assigned Supervising Resource Navigator. 

-Protects and maintains individuals’ privacy and confidentiality. Manages assigned caseload and ensures prompt and accurate data entry and follow-ups with cases and/ or contacts and escalates cases to supervisor, as appropriate. 

-Recognizes, documents and alerts the supervisor of trends in customer calls. 

-Checks records for accuracy of information and for conformity with established policies and procedures. 

-Effectively communicates and demonstrates the System’s acknowledgment and awareness of diverse health – related needs and concerns to patient, family members, caregivers, staff and the community. 

-Performs related work. 

-Call center operating hours are initially planned as (9AM-9PM) seven days per week

Qualifications 

Qualification Requirements: 

  • Two years of coursework or equivalent experience in social or human services or related fields (social work, human services, community/public health, sociology, psychology, mental health counseling, or related field) 
  • Must be a current CUNY student or CUNY Alumn 
  • Familiarity with and experience working in New York City health systems or social/human services. 
  • Candidates preferred to have experience working with community resources and social services in different parts of New York City that can inform connecting patients with COVID and their families to local resources. (Please indicate what location in your cover letter).

POSITION: COVID-Free Queens!, Community Health Associate

Position Summary:

As part of New York City’s efforts to manage the COVID-19 pandemic, the Test and Trace program, or T2, ensures that testing for the virus is widely available and encourages New Yorkers to get tested.  PHS partnered with trusted community-based organizations, collectively called COVID-Free Queens!, who are responsible for distributing  educational materials, PPE, and provide in-person and virtual individual and group health education to thousands of community members as part of a city-wide effort to manage the spread of COVID-19

As this program has been extended through June 2021, PHS seeks a COVID-Free Queens! Community Health Associate to conduct direct community outreach and engagement as part of the Test and Trace efforts. Over the coming months, the City’s Test and Trace efforts will be focused primarily on street outreach and the Community Health Associate will serve a “mobile” role, enthusiastically conducting outreach in the priority and hotspot areas through June 2021.   Under the overall supervision of the Project Manager, the Community Health Associate will work closely with other PHS staff including the COVID-Free Queens! Project Coordinator, the Healthcare Community Partnerships unit, and community-based partners and community members throughout implementation. The Community Health Associate position is up to 35 hours per week.

This project is grant funded and has been extended through June 30, 2021.

Specifically, COVID-Free Queens! Community Health Associate will:

  • Distribute promotional materials (flyers, posters, palmcards) at local businesses, community members on the street, local organizations and institutions.
  • Assist the Project Coordinator to identify appropriate areas in the community to conduct outreach and identify areas that may need to be prioritized based on information provided by the City, word of mouth and/or observations on-the-ground.
  • Engage community members, either at an event, at a local site, or on the street to inform them about ways to prevent the spread of COVID-19 and communicate updated COVID-related messaging as it becomes available.  
  • Attend trainings set up by the COVID-Free Queens! Coalition to improve understanding of and knowledge around current information and messaging on COVID-19 to inform the public.
  • Listen to concerns and questions from the public related to COVID-19 and answer questions in order to stop the spread of misinformation, lessen fears, and ultimately support the community in preventing further spread during the pandemic. 

Qualifications:

  • Must be willing to be outside for up to 3 hours or more at a time during the Winter months on a daily basis, walking and engaging the public

Program Description:

Health Families Bushwick-HFB identifies expecting families in Bushwick, Brooklyn – who experience stress and may be at risk for child abuse and/or poor health and/or child development issues and provides weekly home visiting services. The goal is to help the families enrolled develop positive attachment and interactions with their infants, master good parenting skills, increase their knowledge about child development and the use of health and social services and to improve educational and/or employment status for the parents who are eligible to receive program services until the child enrolls in school.

Position Summary:

Healthy Families Bushwick-HFB, a Program of Public Health Solutions, seeks a bilingual Spanish-English speaking Family Support Specialist (FSS). The FSS  will outreach to families in the community, enroll families in services, and work with the participant families over the long term (3-5 years) to provide Family Support Services. The services are home-based, culturally responsive services. The FSS will be trained to implement an Assessment Tool on which to base the Family Service Plan.  

Specifically, the Family Support Specialist (FSS} will:

  • Outreach to families in the community to inform them about services available including Home Visiting. 
  • Implement the program Screen tool to identify families who meet criteria to participate. 
  • Enroll the families who meet criteria until their caseload is filled.
  • Work with the families on their caseload over the long term until the child enters an educational program (Head Start, Preschool, or Kindergarten).  
  • Provide prenatal, parenting and health education.
  • Promote optimal child development, enhance parent-child relationships and improve family support system through weekly home visits. Administer the ASQ, and other screens.
  • Identify and refer for contacts/appointments at other supportive agencies, including health care appointments.

Qualifications:

  • Associate degree in Early Childhood Development, Health, Social Work or related human service field, preferred.
  • Minimum requirement HS Diploma or GED (documented). 
  • Must be bilingual (Spanish) with strong writing and speaking skills.
  • At least two years of experience home visiting and working with low-income, racially and ethnically diverse urban families, preferred.
  • High level of maturity and experience working with children (0-3 years old) and their families, preferred.
  • Strong knowledge of early childhood development, parenting skills and development of parent-child relationships, preferred.
  • Must be able to work a flexible schedule including some evenings and weekends.

POSITION: Associate Director, Research Partnerships

Within MJFF, Research Partnerships helps get to better Parkinson’s treatments faster by driving two key activities:  

  • increasing engagement and developing partnerships with various stakeholders, including biopharma, government, nonprofits and patients 
  • integrating the patient experience and viewpoint across the research spectrum. This position supports our efforts to engage volunteers in clinical research. 

Position Description:  

Clinical trials are the final hurdle in getting to new FDA-approved treatments. It requires patient-focused experts, leading clinical research teams, and drug developers working in concert to mobilize the community of volunteers needed to participate in research. MJFF is seeking an enthusiastic collaborator with strong project and relationship management skills who can marry these perspectives to develop and execute on both proven and novel approaches to recruiting volunteers for Parkinson’s research. 

As an Associate Director, Research Partnerships, you will be the touchpoint for a cross-departmental internal team, working with Parkinson’s researchers, CROs, and patient advisors who collaborate to make trial recruitment more accessible, faster, less costly, and more efficient. As part of your role, you will help evolve MJFF’s best practices to meet the demands of a changing research landscape, consult with biopharma partners to accelerate their study enrollment, and play a critical role in implementing our recruitment strategy for a landmark study in Parkinson’s disease – the Parkinson’s Progression Markers Initiative (PPMI). Primary responsibilities include:   

Project Management: 

  • Executing strategic action plans and actively managing deliverables 
  • Solicit input and opinions from a variety of advisors with proven track records and translate them into recommendations and actionable steps for MJFF (strategic perspective is critical here) Design and get buy-in for project plans and execute them in an efficient manner 
  • Perform due diligence on new strategies, tactics, and potential vendors; deliver recommendations to leadership and stakeholders, and implement on next steps 
  • Manage inquiries and follow through on requests from biopharma, research centers, and the Parkinson’s community to engage with MJFF’s research engagement resources  
  • Near term, priority project:  
  • Provide strategic insight, solicit buy-in for strategic plans, and execute approaches to engage Parkinson’s patients, care partners, and individuals at risk for Parkinson’s in PPMI, including development and oversight of participant communications, retention incentive programs, and collaborations with referring partners  
  • Develop and maintain dynamic data analytics and reporting on PPMI participant engagement and performance to inform study and MJFF leadership decisions   

Relationship Management: 

  • Develop relationships inside and outside of the Foundation and conduct outreach with the clinical trial and Parkinson’s patient community 
  • Cultivate relationships with clinical trial staff across academic research centers, biopharma and partner organizations 
  • Work with Steering Committee representatives from MJFF-sponsored studies, patient and family Ambassadors, Community Partners and other advisors 
  • Represent MJFF at appropriate meetings, conferences and other venues 

Key attributes of an outstanding Associate Director are excellent decision-making and judgment, creative ‘out of the box’ thinking and strong project and time management skills. An innate sense of urgency as well as an ability to work productively and proactively – both independently and as part of a team – are key. This role reports to the Senior Associate Director of Research Partnerships.  

Job Requirements: 

An understanding of drug development and/or clinical research is not required. Some of our most successful team members come from backgrounds in other industries with a strong functional skill set. The Associate Director role requires a strategic thinker who also wants to be hands-on to support aggressive goals in this arena. The ideal candidate will be flexible and solutions-oriented with a proven track record of managing multiple complex projects and important relationships simultaneously. Excellent oral and written communication skills and presentation skills are a must. Advanced degrees, such as an MA, MBA, MPA and relevant management or strategy experience are preferred. This  will be a remote position to start, however, employees should expect to work out of our New York City office in the foreseeable future. The role is not conducive to telecommuting. The Associate Director must also be willing to travel as needed.  

Ideal candidates will also possess:

Job summary

This Program Associate position is focused on supporting the business process of the foundation’s funding portfolios by playing a unique role on the Research Programs team, defining and delivering strategic and tactical project management expertise. The Officer oversees and manages a portfolio of grants, ranging from one-year to multi-year projects, with individual researchers and investigators, universities, private-public partnerships, and multi-partner consortia. The Officer partners closely with the scientific staff to align delivery of scientific goals and business management procedures. The Officer acts as a primary liaison between external grantee partners and MJFF business partners.

Primary Roles and Responsibilities

  • Actively contribute to strategic planning discussions and process around funding priorities and planning.
  • Ensure MJFF best practices and rigorous project management methods are employed in developing and executing projects
  • Relationship management of external partners, which include other research funders, key opinion leaders, individual researchers and investigators, universities, and private-public partnerships.
  • Coordinate and manage MJFF project committees, working groups, and multi-partner consortia by working closely with internal and external collaborators to develop agendas, coordinate logistics, prepare materials for discussion, circulate minutes, chair meetings as needed and support execution of next steps.
  • Troubleshoot and problem solve as issues arise; exercise excellent decision-making and propose solutions for tough issues related to project administration
  • Serve as the business lead between MJFF science team and grantees to design and manage the operational lifecycle of a grant or funded project.
  • Oversee grants to individual Investigators, universities, and other specialized partners; manages relationship and project delivery lifecycle from proposal process through post-award to close out.
  • Lead and support program evaluation initiatives during and post-grant.
  • Acts as liaison between internal and external stakeholders with internal links in the grants management process, including research partnerships, finance, and other administrative support systems to ensure smooth operational management of the grants process.
  • Facilitate the processing for legal review and/or account setup of grant awards, contracts, supplements, and other award instruments
  • Provide support, narrative, and act as a resource of knowledge for issues occurring throughout the lifecycle of a grant.
  • Leverage grants management software and related applications to track and communicate project progress, and to drive continual process improvement initiatives.
  • Pro-actively communicate foundation processes and procedures to grantees, applicants and other external stakeholders.
  • Routinely review and recommend best practices to improve workflow and systems efficiencies.
  •  Serve as back up and short-term absence coverage support for Officer colleagues.
  • Serve as educator, trainer, and partner for new hires and existing colleagues on context and understanding around program portfolios.
  • Engage in the wider Foundation-community on staff-driven projects and initiatives.

Ideal candidates will possess:

  • Experience in grants life-cycle administration and compliance for varying types of grants and grantees.
  • Demonstrated project management skills, with 2-3 years- experience supporting complex projects; knowledge of and experience in project management tools and methodologies strongly preferred
  • Excellent time management skill required; demonstrated ability to prioritize work at hand and manage multiple projects simultaneously
  • Ability to maintain exceptional attention to detail while considering how those details impact the big picture
  • Strong interpersonal and people management skills; ability to work productively both independently and as part of team, including reporting to cross-functional managers
  • Excellent written and verbal communication and presentation skills
  • Ability to work within a team in-person, remotely or a combination of these setting
  • Strong computer skills including use of standard software packages (e.g., Word, Excel, PowerPoint, etc.) and familiarity with additional technologies useful for team project management.
  • Working knowledge of grant management systems (e.g. Smart Simple, Blackbaud, etc.) preferred but not required
  • Willingness to adapt when necessary; comfortable with ambiguity and flexible to changing business dynamics and priorities
