As you may know, Transitions Clinic Network (TCN) supports a national network of clinics that provide care for individuals with chronic health conditions who are recently released from incarceration. Founded on the idea that the people closest to the problem are also closest to the solution, each clinic that adopts the TCN program employs at least one community health worker (CHW) with a history of incarceration as part of the primary care team; CHWs also work closely with patients to address social determinants of health. Developed in San Francisco in 2006, TCN has supported sites in 14 states and Puerto Rico in implementing the TCN program.
TCN’s California network is growing, and we are excited to announce that we are hiring for two new positions to build out programs ensuring individuals leaving prison are connected with care in their community. This will fill a key gap, as there is currently no established system of care between the prison medical system and community health systems. Here is more information about the two positions:
1) Care Coordinator (New Position): The Care Coordinator will lead TCN’s efforts to build a system of care between California state prisons and community health systems. This position will work closely with staff in the prison system, parole, probation, transitional homes, primary health clinics, and community health workers with lived experience of incarceration in order to develop a system to improve continuity of care. The Care Coordinator will also work directly with individuals with chronic health conditions leaving prison to ensure they are linked to appropriate care in the community. Position Scope
Health & Medicine is currently seeking a full-time Director of Workforce Programs to join our ten-person staff. This position will lead the Chicago Area Health Education Center (AHEC), which is part of a network throughout Illinois that will provide educational ‘pipeline’ programs to address the development and retention of the state’s health workforce. The intent is to improve health care for the underserved through health professions education, health careers development, and community and public health promotion activities. The IL AHEC Network will concentrate its workforce development activities on: 1) improving distribution through recruitment/retention programs; 2) increasing diversity through educational outreach to youth in underrepresented communities; 3) improving quality through regionally-based educational offerings; and, 4) increasing access through developing a diverse and culturally competent workforce. The Network has received funding from the US Department of Health and Human Services Health Resources and Services Administration (HRSA) through August 31, 2012.
In addition to leading the Chicago AHEC, the Director will provide mission oriented and executive leadership to other Health & Medicine workforce initiatives including supporting the Rush Medical Center Geriatric Workforce Education Program, City and County career pathway plans for contact tracers and workforce partnerships, advancement of Community Health Worker policy in Illinois, and other essential functions of Health & Medicine’s workforce policy and program.
Primary Responsibilities include:
- Responsible for the developing a work plan in collaboration with the ED, and ensure all grant deliverables are completed within time and scope
- Manage program budget in collaboration with Finance Director
- Serves as the center’s primary liaison and external representative with multiple organizations, agencies and public officials including the UIC College of Medicine –Rockford and Chicago, the federal AHEC office, the National AHEC Organization, and local, state and federal health and education organizations.
- Partner with and continue to establish working relationships with academic and community-based organizations and providers that provide care to the underserved
- Monitor and collaborate with existing health workforce development programs, including the UIC UMED program and the IL Primary Health Care Association’s SEARCH program, to further the center’s mission
- Coordinate and plan events related to the mission of the Center
- Monitor, track, and evaluate program processes, impacts, and long-term outcomes
- May advise on complex training or curriculum design projects; may lead design projects or supervise multiple events
- Support the development and implementation of the contact tracing earn and Learn program, by providing consultation services to the Recovery Corps Fund and its public sector partners in Chicago and Cook County.
- Identify policy and systems change needed to support and grow the CHW workforce in Illinois
Qualifications
- Bachelor’s degree with 5 years work experience or Master’s degree with 4 years work experience
