POSITION: Population Health Program Interns Fall/Winter 2020 – 2021 (2 positions)
ORGANIZATION: New York City Health + Hospitals, Lincoln Hospital
Overview:
New York City Health + Hospitals is the largest safety net health care system in the country and serves 1.3 million New Yorkers, nearly 400,000 who are uninsured. Lincoln is one the system’s acute care hospitals serving among others, residents of the South Bronx. The hospital carries out various initiatives and performance improvement projects to better the health of its patient population and the South Bronx community. These include projects to improve the management of chronic diseases, such as diabetes and high blood pressure, address social determinants of health, use community health workers to make home visits to patients, and reduce avoidable emergency room visits and inpatient stays.
Internship Description:
We will assign the Program Interns to one of the following projects, based on interest and availability:
- Designing and implementing strategies to address high risk patients (those likely to have a large number of inpatient admissions and emergency room visits in a year)
- Improving health outcomes for patients with diabetes or hypertension, including by connecting patients to resources to address patients’ social needs.
Responsibilities include:
Interviewing staff and patients; attending meetings; conducting analysis of data; preparing charts and presentations; preparing tools such as work flows, posters, flyers, toolkits, flowcharts, and checklists; reviewing literature and best practice; other tasks as needed.
A minimum of 6 hours a week and 2 month commitment is required. Additional hours and a longer commitment is welcomed.

Skills and qualities needed:
Able to work during regular business hours. Please have some knowledge of or ability to quickly learn Microsoft Excel. It is helpful to have some experience analyzing data. Please also have good interpersonal and writing skills.
