POSITION: Implementation Specialist

The HIV Care and Treatment Program seeks an Implementation Specialist (City Research Scientist II) in the Quality Management Unit to guide the delivery of evidence-based approaches to promote quality management and improvement among HIV care and treatment programs, with a particular focus on reducing disparities in viral suppression.

The Clinical and Scientific Affairs Unit is seeking an Assistant Director to direct and lead a citywide hypertension initiative, Take the Pressure Off, NYC! (TPO, NYC!). The Assistant Director will report to the Director of Clinical and Scientific Affairs and lead a team of four people. Hypertension is one of the leading contributors to death and a key driver of health inequities. TPO, NYC! fosters collaborations across the work of multiple stakeholder groups engaged in clinical, community or population approaches to reducing hypertension, to enhance and expand hypertension efforts across the city. The Initiative launched in early 2017 and currently includes more than 100 coalition members, with great potential for broad scale and meaningful population health impact. The Clinical and Scientific Affairs Unit focuses on utilizing evidence and research to implement programming to address cancer, heart disease, and other health concerns through clinical and community partnerships. Under the guidance of the Director of Clinical and Scientific Affairs, the Assistant Director will plan, direct, and oversee the work of the Take the Pressure Off, NYC! (TPO, NYC!) program. Key responsibilities include:
- Engage in visioning for public health efforts to improve hypertension (HTN) recognition, diagnosis, and care in NYC, with an emphasis on eliminating health inequities.
- Lead implementation of program aims and goals, and oversee and guide TPO, NYC! program team. Collaborate as needed with others at the Health Department working on improving prevention and management of HTN.
- With the program evaluator, co-design and oversee research and evaluation activities, including development of evaluation plans and oversight of literature review on hypertension management, connection to care, equity, and related topics.
- Maintain an anti-racist approach to program work, including approaches such as embedding efforts to combat structural racism, consciously acknowledging inequities and seeking to understand the reasons behind them, seeking out and listening to community voices, and working toward best practices for gathering and reporting qualitative and quantitative data.
- Maintain and support relationships with individuals and organizations in the TPO, NYC! coalition, a multi-sector coalition of stakeholders concerned with reducing the impact of hypertension; ensure all sectors are represented, enlist new members who can bring new strengths and perspectives, and ensure relevant data and results are communicated back to members.
- Lead development of an annual HTN Summit, topic- and location-specific learning collaboratives, topic-specific committees within the coalition.

POSITION: Clinical- Community Linkages Specialist

POSITION OVERVIEW
The Clinical-Community Linkages team works with clinical and community organizations to eliminate health inequities related to the prevention and management of chronic conditions. The team works to increasing access to lifestyle management programs by building capacity for organizations to deliver programs and developing referral pathways between organizations. Targeted communities have substantially higher than city average rates of pre-diabetes, diabetes, hypertension and related morbidity and mortality and include populations that speak languages other than English. Culturally responsive programming including place-based strategies and health system collaborations are needed to achieve health equity in these communities.
The Clinical-Community Linkages Specialist will work on building capacity for the National Diabetes Prevention Program in communities within NYC and may support additional self-management programs such as the Diabetes Self-Management Program. This position will report to the Manager of Clinical-Community Program Linkages. The Clinical-Community Linkages Specialist will be responsible for the coordination of clinical and community-based organizations capacity to host and deliver lifestyle management programs with the aim of increasing access to and enrollment in these programs. The Clinical-Community Linkages Specialist will work to connect program providers with community resources to increase their participants engagement in lifestyle change. Resiliency Initiative Project Supervisor
The Resiliency Initiative Project Supervisor will provide oversight of all Resiliency Initiative (RI) activities. The RI provides social support to families served by BHC’s cluster of health centers both domiciled and those experiencing homelessness. The RI targets patients and families of young children 0-5 years of age with the aim of mitigating social stressors, fostering positive parenting and raising healthy and resilient children who are school ready and can reach their full potential.
The RI achieves these aims through a number of programs including:
- Group prenatal and well child primary care
