Under the supervision of an AfterCare Navigator supervisor, the AfterCare Navigator performs remote work in support of H+H’s Test and Trace’s AfterCare Program. AfterCare Navigators survey individuals who were diagnosed with COVID-19 (“Cases”) and have completed their Test & Trace monitoring, but who may require a continuation of assistance. The AfterCare Navigator will connect Cases with identified resources and elevate complex cases to respective supervisors. With support from the New York Community Trust, the Altman Foundation, Samuel Freeman Charitable Trust and New York State Health Foundation, CONVINCE USA, an initiative at the CUNY School of Public Health and Health Policy, has launched the New York Vaccine Literacy Campaign, a vaccine education and engagement initiative that will establish community-based channels for vaccine communication, training, and strategies. NY VLC will increase community-level access to vaccine education and information by offering tailored webinars, education modules, training, and other capacity-building resources. The Campaign operates in NYC, Long Island, Rockland, and Westchester counties.
In consultation with a community advisory board, we will identify vulnerable populations that are at risk for unaddressed questions and concerns about vaccination and therefore more susceptible to mis/disinformation. Publicly available vaccine dosage data, as well as quarterly surveying aimed at assessing knowledge, beliefs, and behaviors around vaccination will inform the tailoring and targeting of content. With the guidance of the community engagement advisory board, the campaign will create inroads to CBOs in need of tools and strategies to build vaccine confidence in their communities.
Capstone/Fieldwork Position Overview:
This role will support the program manager of the NY VLC and help ensure that the team meets greater program goals and deliverables in a timely and effective manner. The position’s responsibilities encompass health communication, content development, and community partner support.
The program assistant role can be filled as:

Job Description
NYC Health + Hospitals’ (H+H) Office of Population Health encompasses chronic disease prevention and management, social determinants of health, population health analytics, and research and evaluation.
The Office of Population Health is currently seeking a Data Analyst to provide support for research and evaluation within the Office of Population Health as well as for the New York University (NYU) – NYC H+H Clinical and Translational Science Institute (CTSI), an innovative research partnership that supports clinical and translational research that transforms health care and improves community health.
Under the supervision of the Senior Director of Research and Evaluation, the Data Analyst will generate data sets and conduct data analysis to advance OPH’s and H+H’s population health research efforts. Advanced technical and analytical skills, strong knowledge of clinical and health care data, critical thinking, and creative problem-solving are essential qualities for this position. The Data Analyst reports to the Senior Director of Research and Evaluation and will work collaboratively across the Office of Population Health, including with other data analysts, as well as with research staff at external organizations.
Responsibilities
- Create secondary data sets using clinical warehouse, registry, claims, and other health care data sources to support new and ongoing research
- Assist internal and external staff with data requests related to priority research questions; collaborate with staff to develop and refine project specifications and scopes
- Perform basic statistical analyses as well as reviews for statistical accuracy, validity, and applicability of analytical methods.
- Translate research and analysis findings into content for briefing documents, presentations, abstracts, manuscripts, and other materials, as needed.
- Liaise and build partnerships within OPH and H+H and with academic partners to support innovative research and analyses to advance population health.

Minimum Qualifications

POSITION: Community Health Worker

POSITION OVERVIEW
The Community Health Worker will serve as a liaison to residents of 6 East/Central Harlem NYCHA housing developments identified as having high rates of poorly controlled asthma, hypertension and diabetes. Community Health Workers will be trained to provide one-on-one coaching sessions to help resident participants implement realistic and culturally appropriate lifestyle changes. Sessions will be held either at the home or at convenient community settings. With shared life experience, Community Health Workers will serve as role models, educate participants about self-help techniques and self-help group processes, teach effective coping strategies and symptom management skills, assist in clarifying rehabilitation and disease management goals and assist in the development of community support systems and networks. Community Health Workers will also directly impact participants’ ability to advocate for appropriate care and healthy neighborhood conditions.

This research project tests the effectiveness of integrating community health workers (CHWs) into primary care practices— in complement to nurse case management and home blood pressure monitoring—to help patients navigate community resources for hypertension control. CHWs will perform outreach, participant recruitment, direct case management, and health education/coaching for Black patients with uncontrolled hypertension. The project will work across a network of 20 primary practices within NYU Langone Health in NYC, in partnership with an established Community-Clinic-Academic Advisory Board and HealthFirst (NYC’s largest Medicaid payer) in two phases. The study’s findings will provide a practical and sustainable system that harnesses existing clinical and community resources to build capacity for primary care practices to manage hypertension control in minority populations.

POSITION: Senior Safety Engineer

Responsibilities
Under the direction of the Supervisor, Safety Management, the selected candidate will be responsible to critically evaluate operational conditions at Port Authority facilities; identify and characterize potential safety conditions; establish recommendations for improvements, develop safety procedures and standards, evaluate safety programs, provide technical advice on accident prevention, evaluate and recommend new safety initiatives and equipment necessary to enhance the overall Port Authority safety program, generate statistical reports review and interpret safety regulations, prepare reports on assigned special projects.
