DESCRIPTION:Learn about SOMOS Innovation – the next step on the path to culturally competent Value-Based healthcare. They are looking to hire new team members in 2021 across many different departments. By utilizing the SOMOS Community Care model\, SOMOS physicians reduced preventable hospital admissions by more than 20 percent\, and avoidable readmissions by 25 percent. Given this remarkable performance\, New York State’s Value-Based Payment (VBP) program designated SOMOS IPA as a VBP Innovator; SOMOS IPA is the only physician-led organization to earn that distinction. SOMOS Community Care is a not-for-profit organization comprised of a network of more than 2\,500 independent physicians that care for nearly one million Medicaid patients\, predominantly of Hispanic and Asian ethnicity in 4 boroughs of New York City and Nassau County. \n
