POSITION: eConsult Data Analyst

ORGANIZATION: NYC Health + Hospitals

SUMMARY: Within the Office of Population Health at NYC Health + Hospitals, the eConsult Data Analyst will support initiatives aimed at improving patient and primary care provider access to specialty care expertise. This role will provide data analysis support for a pilot initiative seeking to implement an electronic referral/consult solution at a selection of NYC Health + Hospitals facilities to improve the patient and provider experience, reduce barriers to specialty care, and improve the quality of care for our patients. This is a new program with the potential to have a substantial impact on health care delivery, and we are seeking an enthusiastic and creative team member to collaborate on impactful ways to share data to drive improvement.

RESPONSIBILITIES: Under the supervision of the Program Manager for Ambulatory Specialty Care and within the framework of the Data Core team, the eConsult Data Analyst will serve as the data analytics support for the eConsult program.

Responsibilities:

  1. Data Liaison: the eConsult Data Analyst will work closely with the eConsult team, clinical site contacts, IT teams, and others as needed to track and monitor eConsult data, proactively identifying anomalies and working with team members to identify and fix issues affecting data quality. The eConsult Data Analyst will also work closely with the Office of Population Health’s Data Core team, participating in broader healthcare analytics projects and sharing expertise in pursuit of the team’s goals.
  2. Data Management and Reporting: the eConsult Data Analyst will be the core resource for the program on all data needs, with particular emphasis on identifying and operationalizing ways to use data to drive process improvements. This may include creation and maintenance of facility-facing eConsult dashboards, ad-hoc analyses to explore areas of interest, and partnering with other members of the team to create tools and resources for facilities. The eConsult Data Analyst will serve as the reporting lead for the project and be responsible for driving report development from conception to reality, including: developing report specifications to meet the needs of the eConsult team and clinical sites, working with H+H IT teams to create reports, conducting QA for new and existing reports, and running routine reports from a variety of data sources, including Epic and Healthcare Intelligence.
  3. Data Analytics: the eConsult Data Analyst will perform basic data analytics to further eConsult program goals, including but not limited to correlation, chi squared tests, student’s t tests, test for trend, Wilcoxon rank-sum tests, scatter plots, line graphs, bar charts, box and whisker plots.

 QUALIFICATIONS: Minimum Qualifications

  1. Bachelor’s degree with a focus on public health, health care, pre-med, mathematics or related field.

 Department Preferences

  • Experience with or exposure to data analysis in Microsoft Excel and/or SAS software.
  • Experience with or exposure to project management, and proven track record of successful interpersonal engagement
  • Demonstrate initiative, sound judgement and flexibility; possess effective time management and organizational skills; and be able to work under deadlines and on multiple projects simultaneously.
  • Position requires excellent verbal and written communication skills as well as strong quantitative interest.
  • Detailed oriented and a commitment to innovation.

POSITION: eConsult Training Coordinator

ORGANIZATION: NYC Health + Hospitals

SUMMARY: Within the Office of Population Health at NYC Health + Hospitals, the eConsult Training Coordinator will provide in-person support and training for the eConsult program, aimed at improving patient and primary care provider access to specialty care expertise. Over the next 2 years, this pilot initiative seeks to implement an electronic referral/consult solution at a selection of NYC Health + Hospitals facilities to improve the patient and provider experience, reduce barriers to specialty care, and improve the quality of care for our patients.

RESPONSIBILITIES: The eConsult Training Coordinator is responsible for developing and delivering support services to NYC Health + Hospitals facilities implementing the eConsult initiative, and will work in coordination with the eConsult team to develop a training program, curriculum, and materials for the eConsult technology as well as best practices in referral management and triage.

Support will include, but is not limited to:

  • Conducting assessments of referral and scheduling workflows and processes.
  • Providing guidance and developing supportive relationships to enhance facility engagement with eConsult and referral management.
  • Working with and advising facility teams to identify and implement process improvement and workflow redesign solutions to overcome challenges and achieve improvements in patient access to specialty care expertise.
  • Developing and delivering trainings tailored for:
    • Primary Care Providers
    • Specialists
    • Clerical/scheduling staff

The eConsult Training Coordinator will coordinate with Office of Population Health and facility staff and leadership on related initiatives in quality improvement and practice transformation, and will work closely with facility teams to promote, advise on, and conduct in-service trainings, grand rounds, workshops, and other activities as needed to engage providers and staff in the eConsult initiative.

QUALIFICATIONS: Minimum Qualifications

  1. Undergraduate degree required; graduate degree preferred
  2. At least three years’ experience with progressive responsibility in program management, quality improvement, ambulatory care management and/or administration, or a satisfactorily equivalent combination of education, training, and experience.

Department Preferences

Experience in health care delivery systems redesign to support improved operations and patient flow.

Demonstrate initiative, sound judgment and flexibility; possess effective time management and organizational skills; and be able to work under deadlines.

Position requires excellent verbal and written communication skills as well as quantitative skills.

Detailed oriented and a commitment to innovation.

Strong interpersonal, communication, and group facilitation skills are essential.

POSITION: Supervisor I, Structural-Level Change/Project Accept LGBTQ Youth (ALY)

ORGANIZATION: CAMBA

SUMMARY: CAMBA’s Project Accept LGBTQ Youth (ALY) consists of several components to promote structural change around family acceptance of LGBTQ youth:   an intervention that draws on a community identification process; the development and dissemination of role model stories and resource kits; group presentations and a parent support group; and a social marketing campaign.   All components utilize stories of acceptance from supportive parents and families of lesbian, gay, bisexual, transgender, and queer/questioning (LGBTQ) youth and teens in order to change underlying causes of HIV infection among this group. The ultimate goal is to promote parental and familial acceptance of LGBTQ youth, thus increasing self-esteem and self-worth and decreasing risk of future health or mental health problems including HIV infection and substance abuse.  By encouraging parents and family members of LGBTQ youth to become role models and share their stories of acceptance, we will help break the silence, remove stigma, and motivate change throughout the community.  Our program targets parents and families of LGBTQ youth, ages 14 to 26, in the Flatbush community, the majority of which are Black or Latino, and surrounding neighborhoods in Central Brooklyn.

RESPONSIBILITIES: The person filling this position is expected, under general direction, to ensure the smooth day-to-day running and supervision of program-related peers and services in accordance with all program goals, targets, and performance outcomes and all CAMBA and program policies, procedures, and protocols; etc.

POSITION: Client Advocate I, Young Men’s/LGBTQ Health Project (YMHP)

SUMMARY: CAMBA’s Young Men’s/LGBTQ Health Project (YMHP) aims to reduce transmission of HIV among young men who have sex with men (YMSM) ages 13-29, including those who identify as transgender or gender nonconforming, with a focus on communities of color in Brooklyn.  YMHP will use a strength-based, youth development approach to provide culturally competent and comprehensive prevention and support services, including: client recruitment through enhanced  and new media outreach; health education and risk reduction counseling; linkage to HIV, STD, and HCV testing and screening; Evidence Based Interventions (EBIs), including Healthy Relationships; VOICES/VOCES; Many Men, Many Voices; and Anti-Retroviral Treatment and Access to Services (ARTAS)] a PrEP support program; linkage, navigation, and retention to care; linkage to supportive services; and condom education and distribution. YMHP will provide services to 300 participants, 25% of whom will be HIV positive.

RESPONSIBILITIES: The person filling this position is expected to: (1) assist program staff in facilitation of EBIs; (2) assist program staff by being a client liaison with outside organizations regarding such matters as education, healthcare, social services, etc; (3) assist clients in attaining their goals by identifying and locating community resources for clients and by making referrals to appropriate services both within and outside CAMBA; including the following:
