 Functional Job Description: Data Manager, Patient-Centered Medical Home (PCMH)

The Data Manager will enable PCMH recognized primary care practices to track, report and communicate performance metrics in order to meet internal or external requirements for accurate data. Duties will include data entry, collection, reporting, analysis, quality assurance and presentation, including but not limited to data required for access monthly reports, Collaborative Care, preventive services, and the NCQA PCMH designation. The Data Manager will work closely with HHC Central Office Medical and Professional Affairs programmatic staff to conduct quality assurance of data and ensure that reports adhere to standardized specifications. Data Manager will report to the Facility Ambulatory Care Director. This is a time-limited grant funded position.

Specific responsibilities will include:

Care Delivery Performance and Utilization Reporting  – The Data Manager will monitor metrics on panel management, continuity of care, access, service utilization, and other key performance domains as outlined in standardized internal and external performance reports. S/he will compile and synthesize data from chart reviews, logs, audits and other data collection methodologies; run and synthesize reports including SQL reports from various HHC data sources including panel management system, scheduling systems, clinical data warehouse and registries; maintain up-to-date and accurate repositories of data; maintain facility, team and provider level data dashboards; and maintain and improve data collection tools.

Population Health Outcomes Reporting – The Data Manager will develop, compile and/or run standardized reports to track clinical outcomes for patients at the patient, provider, team, program and clinic level.

Grants and Recognition/Accreditation Program Reporting – The Data Manager will become versed in NCQA PCMH Standards, Collaborative Care reporting requirements and other programmatic reporting requirements. S/he will be responsible for implementing these reporting standards and ensure accurate and timely submission of necessary data.

Quality Assurance – The Data Manager may collaborate with IT and other departments to ensure and promote accurate data capture and validation. S/he will help to develop and manage database applications to streamline administrative and data gathering processes. S/he may provide oversight and/or training to clinic staff involved in data collection activities to maintain data quality.

Eligibility Criteria: Candidate should have at least 3 years of data analyst or management experience. Degree in Public Health, Statistics, or related field required. Experience in public health, healthcare administration, quality improvement preferred. Knowledge of process evaluation, quality assurance and ability to work with interdisciplinary teams is essential. Candidate must be have strong Microsoft Excel skills and may be required to pass a skills test.
